Belfry Nights Registration

Each student participating in Belfry must be checked in by an adult and remain on campus until checked out by an authorized adult no later than 8:00pm.
Students are not permitted to have cell phones during activities. Cell phones will be checked in at the Registration Station and may be requested to communicate with parents and guardians during the night.
Family Information
Emergency Contact- Please list an adult that is different than the Guardian named above.
Authorized Drop Off/Pick Up- All students must be checked in and out by an ADULT.

Student Conduct Agreement
I understand that Belfry Night is a fun and safe event for everyone attending. By signing below, I agree to:
  • Show respect to all attendees, staff, and the campus of Dade City First UMC at all times.
  • Avoid any inappropriate language or behavior.
  • Stay inside the event area unless given permission to leave by a chaperone.
  • Follow all directions given by event organizers and chaperones.
  • All alcohol, tobacco, vaping devices, drugs, and weapons are prohibited.
I understand that failure to follow these rules may result in being asked to leave the event and that my parent/guardian will be contacted.
By signing this agreement, I acknowledge that I have read, understand, and agree to follow all of the above rules.
